Making the switch to multifocal lenses is a significant step towards a more natural and comfortable vision experience. 

If your optician or optometrist has prescribed or suggested multifocal lenses, it means you could really benefit from them on a daily basis.

Multifocal lenses (also called progressive lenses or varifocal lenses) offer a gradient of vision for near, intermediate and far distances, all in the same lens – with no visible line between different prescription powers! 

Of course, you may need some time to adjust to these new lenses at the start. Please know that this is completely normal. 

The brain and eyes are adapting to a new way of seeing and like any adjustment, it requires a bit of patience and practice.

The adjustment period of new multifocal lenses

The adjustment period to multifocal lenses varies from person to person, ranging from a few days to a couple of weeks. 

During this time, it’s common to experience some initial challenges. 

As your eyes get used to the gradient zones of the lenses, you might notice that your vision feels unusual. 

This often includes blurry vision when shifting focus between objects at different distances, such as when moving your gaze from near to far objects or vice versa.

Start in a familiar environment

To effectively adjust to multifocal lenses, it’s recommended to start in a familiar environment. 

Begin by wearing your new lenses in the comfort and safety of your own home, where you can navigate and adapt at your own pace.

Engage in familiar tasks such as reading a book or watching television. 

These activities are excellent for gradually getting accustomed to the multifocal lens gradient, as they allow you to test various focal lengths in a controlled and comfortable setting.

Eye and head positioning

Adapting to multifocal lenses involves precisely positioning your eyes and head for near, intermediate and distance vision.

For near vision tasks like reading, lower your gaze, not your head, towards the bottom section of the lenses. 

This part of the lens is designed for close-up work and using just your eyes to shift focus ensures you utilise the correct zone.

For intermediate vision, such as when working on a computer screen or looking at something at arm’s length, aim to look straight ahead through the middle part of the lenses. 

This requires a slight adjustment in eye level without significant head movement, aligning your gaze with the intermediate zone of the lenses.

When focusing on distance vision, like watching television or driving, it’s important to look through the upper part of the lenses.

Lift your gaze slightly while keeping your head in a natural, upright position. 

This aligns your eyes with the top section of the lenses, which is optimised for far distances.

Don't switch between your new and old pair

When you start wearing multifocal lenses, it’s important that you do not switch between your old and new glasses. 

Consistency is vital for achieving clear vision and adjusting to the new visual dynamics. 

The more you wear your new glasses, the faster your eyes and brain will adjust to the new lenses, leading to clearer and more comfortable vision.

Incorporate visual exercises into your daily routine to accelerate this adjustment. 

Regularly practice focusing on objects at varying distances to train your eyes and achieve optimal vision correction.

Have you ever heard of eyelash mites? They may sound like a cross between a horror movie villain and an unexpected house guest, but these tiny critters are not as scary as they seem. 

A natural part of the human microbiome, these minuscule creatures can usually coexist without causing any harm.

What are eyelash mites?

Eyelash mites, scientifically known as Demodex folliculorum and Demodex brevis, are microscopic parasites that inhabit human hair follicles, particularly in the eyelash area.

Demodex folliculorum is the most common type of mite and lives within hair follicles on human skin. They are mostly found on the face and are most prevalent around the eyes. 

Demodex brevis is another kind of mite found on humans. These mites are so small they cannot be seen with a naked eye. 

They only become noticeable and cause reactions or problems if they are present in large quantities. 

While all they want to do is feed on dead skin cells, when their population grows, and more eyelash mites are present, it can lead to various eye-related issues and discomfort. 

Understanding the symptoms, causes, treatments and prevention methods associated with eyelash mites is crucial for avoiding eye and skin problems.

What causes eyelash mites?

The human body is home to many microorganisms, and eyelash mites are one of them. 

Generally, their presence in small quantities can be beneficial as these microscopic mites feed on dead skin and help by removing excess oil and ridding the body of dead skin cells.

The exact cause of an eyelash mite infestation is not always clear, but factors that might contribute to their proliferation include:

  • Poor hygiene: inadequate cleaning of the eyelids and face can allow mites to thrive.
  • Oil gland irregularities: overproduction of oil in the eyelid glands can create an environment conducive to mite growth.
  • Weak immune system: individuals with compromised immune systems may be more susceptible to mite infestations.
  • Age: mite populations tend to increase with age, as the body’s ability to control their numbers may decrease.
  • Skin conditions: those with dry or flakey skin, or rosacea, are prone to also have eyelash mites. 

Eyelash mites are contagious and can be passed on through close contact with someone who has them. The affected person may have adult mites or larvae that can spread from person to person. 

They can also be passed through mascara or other eye makeup, cosmetic tools and face towels, so avoid sharing these items.

Symptoms of eyelash mites

The presence of eyelash mites might not always produce noticeable symptoms. 

When severe infestations occur and too many eyelash mites are present around the eyelash follicles, symptoms may then occur. 

Some of the common symptoms of eyelash mite infestations include:

  • Itching: persistent itching around the eyelashes is a common symptom caused by the movement of the mites.
  • Redness: increased redness and inflammation of the eyelids.
  • Burning sensation: some individuals may experience burning or stinging in the eye area.
  • Crust formation: crust or debris formation at the base of the eyelashes, particularly upon waking up.
  • Eyelash loss: excessive loss or thinning of eyelashes due to mite activity.

If you are experiencing uncomfortable symptoms and suspect an eyelash mite outbreak, you should see your eye doctor immediately.

How to treat eyelash mites

It’s essential to approach the treatment of eyelash mite infestations cautiously. 

Abrasive or harsh cleaning methods can irritate the eyes, so it’s best to consult with your eye doctor to ensure an appropriate and effective approach to managing eyelash mite infestations.

The treatment for an eyelash mite infestation typically involves a combination of hygiene practices and, in some cases, medicated solutions prescribed by a healthcare professional.

For a mild infestation, healthcare providers might prescribe medicated cleansers or scrubs formulated to eliminate mites and their eggs. These products are designed to target the mites without irritating the eyes.

In the case of a severe infestation or risk of secondary infections due to mite activity, an optometrist may perform a deep cleaning of the eyelids. 

They might also prescribe antibiotics or other medications to address any infections.

Always follow the advice and treatment plan recommended by your eye doctor. They can provide personalised guidance and medications tailored to your specific situation.

How to prevent eyelash mites

Preventing eyelash mite outbreaks involves several key practices. Maintaining good hygiene is the first step. 

Regularly clean your eyelids and face using a mild cleanser. Do not share products like face towels or eye makeup and tools, as this can spread mites. 

Washing your bedding regularly can also reduce the risk of an eyelash mite infestation. Clean pillowcases and bed sheets minimise the risk of mite transfer.

While eyelash mites are common, an overpopulation of these microscopic organisms can lead to discomfort and eye-related issues. 

Regular hygiene practices, medical intervention when necessary and maintaining a healthy lifestyle are critical steps in managing and preventing complications arising from eyelash mite infections.

Our eyes are complex ecosystems that depend on a delicate balance of moisture, oil and a smooth ocular surface to maintain clear vision and comfort.

A key player in this balance is a set of small glands known as meibomian glands, which keep the eyes lubricated and healthy.

When these glands malfunction, they can lead to a condition known as Meibomian gland dysfunction, which can disrupt the eye’s tear film, resulting in real discomfort.

As one of the main causes of dry eye syndrome, Meibomian gland dysfunction is a common disorder that’s often overlooked.

In this article, we’ll explore its symptoms, causes and the various treatment options available to manage and treat this condition.

What are meibomian glands?

Human meibomian glands were named after the German doctor Heinrich Meibom, who first described them in the 17th century.

These tiny glands are embedded in both the upper eyelid and the lower one, strategically positioned along the eyelid margins.

Their main function is to secrete a lipid-rich substance called meibum, which forms the top layer of the tear film (a multi-layered fluid coating on the surface of the eye) and is essential in reducing tear evaporation (Chhadva et al. 2017).

This lipid layer is a complex mixture of oils, including phospholipids, cholesterol, free fatty acids and a variety of other lipids, which helps to create a smooth optical surface on the cornea, facilitating clear vision.

In healthy eyes, each blink spreads the meibum across the ocular surface, forming a protective barrier that stabilises the tear film and maintains ocular hydration.

This lipid barrier is not only crucial for comfort but also for protecting the eyes from microbial invasion and environmental irritants.

What is meibomian gland dysfunction?

Meibomian gland dysfunction is an ocular surface disease where the quality or quantity of human meibomian gland secretions is affected. 

Specifically, it arises when the meibomian gland orifices become obstructed, when the glands do not secrete enough oil or the quality of the oil is altered.

Such dysfunctions can lead to compromised meibomian gland function, resulting in abnormalities in the tear film and adversely affecting the ocular surface. 

This condition often manifests as discomfort and can lead to a range of vision problems, including irritation, blurry vision or the development of dry eye syndrome.

Different types of meibomian gland dysfunction

Human meibomian gland dysfunction can present in different forms.
 
Your eye doctor will need to carry out a comprehensive analysis and take into consideration underlying causes or contributing factors, such as inflammatory conditions, hormonal changes or environmental influences.
 
The two primary types of MGD are:
 
  • Hyposecretory MGD: This type occurs when the meibomian glands do not produce enough oil or the oil they produce is of poor quality. As a result, the tear film lacks the necessary lipid layer to prevent evaporation, leading to symptoms of dry eye.
  • Hypersecretory MGD: In this form, the meibomian glands produce an excessive amount of oil. This can lead to an unstable tear film and symptoms similar to dry eye, as excessive oil can disrupt the delicate balance of the tear film’s layers.

Additionally, MGD can also be classified based on the physical state of the meibum and the presence of any blockages in the glands:

  • Obstructive MGD: This is the most common type, where the glands become blocked, typically by thickened oil or keratinised debris. This obstruction prevents the normal flow of oils into the tear film, leading to evaporative dry eye symptoms.
  • Non-obstructive MGD: The glands are not physically blocked, but there may be issues with the quality or quantity of the oil being produced.

What are the symptoms of meibomian gland dysfunction?

The symptoms of meibomian gland dysfunction may include:

  • Dryness and discomfort in the eyes
  • A gritty or foreign body sensation as if something is in the eye
  • Itchy, red or swollen eyelids
  • Fluctuating blurry vision that often improves with blinking
  • Crusting of the eyelashes or eyelid margins, particularly noticeable upon waking

These manifestations are commonly associated with symptomatic meibomian gland dysfunction, emphasising the need for timely diagnosis and management to alleviate discomfort and preserve eye health.

What are the causes of meibomian gland dysfunction?

The root causes of meibomian gland dysfunction can be diverse, ranging from blockages of the oil gland openings due to thickened secretions or external factors that can alter the lipid layer of the tear film.

Some of the causes include:

  • Ageing, as gland function tends to decrease with age
  • Hormonal changes, especially during menopause
  • High lipid levels in the blood
  • Inflammatory skin conditions, like rosacea
  • Allergic  conjunctivitis
  • Heavy make-up, as using eyeliner can block the meibomian glands

MGD risk factors

Certain factors may increase the risk of developing MGD, such as:

  • Extended screen time, leading to reduced blinking frequency
  • Environmental factors such as windy, dry or dusty conditions
  • Wearing eye makeup, which can block gland openings

How is meibomian gland dysfunction diagnosed?

Diagnosing MGD involves a comprehensive eye exam.

An eye care professional may use specialised equipment to examine the eyelids and the quality of the tear film and flow of the oil in the meibomian glands.

Non-invasive imaging techniques like meibography can also assess the structure of the glands.

How to manage and treat meibomian gland dysfunction

Management and treatment of MGD may include:

  • Warm compresses: applying a warm compress to the eyelids (5 minutes daily) can help melt the thickened oil, allowing better flow from the glands.
  • Eyelid hygiene: regular cleaning of the eyelid margin can reduce blockages and improve gland function.
  • Medications: certain medications, including topical antibiotics or steroid eye drops, can help control inflammation. Although MGD is not an eyelid infection, antibiotics can help with the inflammation and bacteria that cause MGD.
  • Nutritional supplements: omega-3 fatty acids can improve the quality of the gland’s secretions.
  • In-office procedures: eye care professionals may perform procedures to manually express the glands or use devices to unclog the oil gland openings. An example is the innovative intense pulsed light (IPL) treatment.
  • Punctal plugs: insertion of tiny devices in the tear ducts can slow the drainage of tears, keeping the eyes more lubricated.
  • Avoiding aggravating factors: taking breaks during screen time and avoiding eye makeup can all help alleviate symptoms.

It is essential to consult with an eye care professional for a proper diagnosis and treatment plan if you suspect you have MGD.

Addressing the condition promptly can prevent further complications and improve quality of life.

They’ve long been used as a home remedy because the compress provides moisture and heat to your eyes, relieving the symptoms of mild conditions like itchiness, irritation, swelling and dry eye, among others.

What can I use a warm compress for?

So, what is a warm compress? It can be moist or dry, typically a cloth or heat pad, that you place over your eyes to ease your discomfort.

There are various mild eye conditions that warm compresses may be used for, such as:

Applying a warm compress to your eyes is great for relaxation! It can help relax your muscles leaving you feeling soothed and calm.

Types of compresses

There are two main types of warm compresses: dry and moist.

A dry compress is achieved using a heating pad, or a hot-water bottle. Whereas, a moist compress requires using a wet towel or washcloth in warm water.

Both types deliver heat, however moist compresses have been found to be more effective in relieving pain and providing faster relief than dry ones.

Nonetheless, if you need a convenient fix that requires zero cleanup, a dry compress can still provide some relief. 

Benefits of a warm compress on your eyes

Now that you know what a warm compress is, here are some of the benefits that it can provide:

  • Adds moisture: moisture can open up the oil glands to improve their function which helps your eyes with normal tear production. 
  • Prevent oils from clogging: when the oil glands aren’t working properly and become clogged, it can result in blepharitis, sties or cysts. The moist heat can clear the obstructed glands and normalise oil flow to the eyes. 
  • Relieves pain: a warm compress provides quick relief by stimulating healing in the area.
  • Eye muscle spasms: the moist heat can help relax the muscles in your eye.

How to make a warm compress for your eyes

 Making warm compresses at home is simple and requires few resources. 

  • Fill a clean bowl with warm clean water. This can be done using the warm water from the tap, or heating it in the microwave. The temperature should be between warm and comfortably hot. 
  • Soak a clean washcloth in the warm water until it’s fully soaked through. 
  • Remove the washcloth and wring out all of the excess water.
  • Fold the washcloth in half or until it is a size that sits comfortable over your eyes.
  • Leave the cloth on your eyes for a few minutes or until it gets cold. 
  • Repeat steps 2-5 for no longer than 10-15 minutes for optimal results. Excessive time can dry out your skin.

Note if you have any infection or discharge from the eye, please change the water in between to avoid further spread of infection.

Remember: the water should never be too hot as it can cause injury to your eye area!

Are warm compresses safe for your eyes?

Warm compresses are safe, as long as you are careful.

The main issue that can arise is the washcloth being too hot which can cause injury.

The temperature of the water should never be uncomfortable or inflict pain. 

Furthermore, warm compresses are not ideal to use on a fresh injury, especially near open wounds.

A cold compress may be more beneficial in this case, to relieve pain and reduce inflammation. 

Lastly, frequent use can dry out the skin around your eyes. Though it is not dangerous, it could leave your skin feeling uncomfortable.

Warm compress to treat dry eye

One of the main conditions a warm compress is used to relieve dry eye syndrome.

Did you know that about 40% of adults suffer from varying degrees of dry eye conditions?

Dry eye is the result of the inability to produce high quality or sufficient amounts of tears to lubricate the eyes.

There are many causes of dry eye syndrome such as age, allergies, lack of sleep, contact lenses, air pollutants and medications.

Dry eye symptoms

There are several dry eye symptoms you may experience, including:

  • Excessive tearing
  • Blurry vision
  • Redness
  • Gritty sensation
  • Stinging or burning
  • Light sensitivity

A compress may relieve these dry eye symptoms as it will add moisture and heat to help tear production and clear the meibomian glands.

The increased blood flow can also alleviate pain and swelling.

This is one remedy among many, others may involve making dietary or environmental changes or prescription eye drops.

Remember, it’s important to consult an eye expert immediately if remedies do not work, or worsen your symptoms.

Using an eye compress can provide quick relief at home, though it may not be what’s best for you if your symptoms are more severe.
